Research overview

Cagrilintide is a modified analogue of amylin, a pancreatic peptide hormone. Its 37-residue chain contains a disulfide bridge between the cysteines at positions 2 and 7 and ends in an amide. At the other end, the amino-terminal lysine carries a C20 fatty diacid through a γ-glutamic acid linker. The development paper describes the chemical design of this lipidated analogue and the structure–activity work that led to its selection. It identifies amylin’s tendency to form amyloid fibrils as a central challenge for the design programme [1].

A later study investigates how Cagrilintide engages both amylin and calcitonin receptor systems. Researchers determined cryo-electron microscopy structures of complexes containing Cagrilintide, the AMY1R or CTR protein, and the signalling protein Gs. They combined structural observations with functional signalling measurements. The reported structures place phenylalanine at position 23 near the transmembrane bundle and identify an internal glutamate–arginine salt bridge, while the terminal proline contacts the extracellular domain [2].

Both complexes examined in the structural study contain Gs. The analysis compares peptide contacts at the transmembrane bundle and the extracellular domain in AMY1R and CTR. Within the peptide, the salt bridge joins glutamate at position 14 to arginine at position 17, and the terminal proline is residue 37 [2].
